A male infant, weighing 2600 grams, was born by vaginal birth at 41.1 weeks gestation The infant was SGA but otherwise, on the first exam, appeared normal. Then, while straining, a bulge was noted where the anus should be. With a more careful look, no anal opening was noted on the physical exam leading to a diagnosis of imperforate anus.
